Access & paperwork
Care should be reachable. When documentation is clinically appropriate, we complete it carefully — never as a shortcut around an evaluation.
Insurance
Avenelle accepts major commercial insurance. Benefits are verified before your first visit so you know what to expect.
Coverage varies by plan, state, and service. Some plans require a referral or prior authorization. We will tell you what we find before you are seen.
Add your contracted plans here before launch (for example Blue Cross Blue Shield, Aetna, Cigna, UnitedHealthcare).
Self-pay
If you prefer not to use insurance, or we are out of network with your plan, cash-pay / self-pay visits are welcome.
A superbill can be provided so you may seek out-of-network reimbursement from your insurer when those benefits apply.
Fees can be listed here once you set them.
Leave paperwork
Family and Medical Leave Act (FMLA) paperwork may be completed for established patients when clinical findings support it and applicable federal and state requirements are met.
FMLA certification is not offered at a first visit and is never guaranteed. Eligibility is a clinical and legal determination. Employers and the Department of Labor — not Avenelle — decide whether leave is approved.
State-specific family leave rules are considered for the state in which you receive care (Texas, Idaho, Washington, or Oregon).
School & testing
For children, teens, and students, we complete supporting clinical documentation when an evaluation supports the need. Schools and testing bodies make the final accommodation decision.
IEP: We provide psychiatric supporting documentation for Individualized Education Program (IEP) teams — history, diagnosis when established, functional impact at school, and recommended supports. Avenelle does not write the IEP itself; the school district develops and implements the plan.
Section 504 and other exceptions: We also complete 504 paperwork and ADHD-related accommodations when clinically supported, including classroom and testing exceptions.

Paperwork requests are reviewed case by case. A requested form is not a promise that leave, a 504 plan, or an exam exception will be granted. Crisis and same-day disability determinations are not provided.

A psychiatric mental health nurse practitioner committed to clinically grounded care that remains deeply human. Cynthia cares for children, adolescents, and adults.
Her clinical background includes psychiatric emergency care, critical care, healthcare leadership, and telehealth nursing—experience that shaped her calm, thorough, and safety-minded approach. She works collaboratively, explains treatment choices plainly, and believes good psychiatric care should help people function better in the life they actually live.
The first visit
The first appointment is an evaluation, not a rushed prescription visit. We use the time to understand your story well enough to make the next step safe and useful.
You will receive intake forms and consent documents. Complete them in a quiet place. Have your medication list, pharmacy, insurance card, and prior records nearby if you have them.
Visits are secure video. Plan to be in Texas, Idaho, Washington, or Oregon for the appointment. Use a private room. For children and teens, a parent or guardian is part of the visit.
We talk through current concerns, how you are functioning at home, school, or work, health history, sleep, safety, strengths, and what you want to be different. First visits are typically unhurried — often 60 to 90 minutes.
You leave knowing the working impression, whether medication is being considered, what to watch for, and the follow-up plan. A first visit does not guarantee a prescription, FMLA paperwork, an IEP, or a 504 plan.
Symptoms and when they started. School, work, and relationships. Medical history, pregnancy when relevant, sleep, substances, and prior mental health care. Safety. Goals that matter to you — not only a diagnosis name.
Tell us at the first visit if an IEP, 504, ADHD accommodation, testing exception, or future FMLA request may be needed. Documentation is completed only after there is enough clinical information. FMLA is for established patients who meet state requirements.

No. Avenelle is a psychiatric-mental health advanced practice nursing practice led by Cynthia Love, APRN, MSN, PMHNP-BC. Services are provided within applicable state licensure, prescriptive-authority, and professional nursing requirements.
Yes. Avenelle is designed as a telehealth-first practice. You must be physically located in a state where your clinician is licensed at the time of the visit: Texas, Idaho, Washington, or Oregon. Availability may vary by state and service.
No. Medication is one possible part of care, not a guaranteed outcome. Recommendations follow a complete assessment, shared decision-making, clinical appropriateness, and applicable laws.
Controlled medications may be considered only when clinically appropriate and legally permitted. Decisions require clear treatment goals, careful monitoring, and regular reassessment. A first visit never guarantees a prescription.
